Thank you for your interest in working with Focus On You Private Care! To schedule an interview please complete the application below and submit the following documents.

To expedite the process, please ensure all documents are full, clear pictures or scans. If your documents are too large for our application form, please submit them to our email address - info@focusonyoupc.com.

Please plan to have the following documents ready

• CNA card (if you have one)
• Resume or a list of your work history (10 years or as much history as you have)
• Covid-19 Vaccination Card (if you have one)
• 3 professional references (name, phone #, job title - include at least 1 supervisor)
• TB skin test results - done within the last 12 months
• 2 IDs for employment verification (acceptable forms of ID https://www.uscis.gov/i-9-central/acceptable-documents)
• CPR / BLS card or other certifications
• Proof of auto insurance (if you are open to allowing clients to ride in your car)
